PLEASE use THE FOLLOWING PROCESS TO REFER A NEW CUSTOMER TO aLLIES:

NOTE: We have temporarily paused DSA referrals for Eugene/Springfield while we work through our waiting list. We’ll announce when DSA referrals reopen via e-mail. Click here to be added to our e-mail update list.


1) Complete the appropriate referral form.

a) If the customer is seeking Attendant Care (1:1) or Attendant Care (1:1) and DSA (groups/classes) in the Eugene/Springfield area, use this form.

b) If the customer is seeking only DSA (groups/classes) in the Eugene/Springfield area, use this form.

c) If the customer is seeking supports (attendant care or DSA) in Florence/at the coast, use this form.

d) If the customer receives Attendant Care (1:1) support from a family member, use this form.


2) A coordinator from Allies sets up an intake meeting/interview with the customer (and their family/home staff, if applicable).


3) Allies coordinator contacts the case manager by email to request service agreement and isp documents. All documents should be sent to Heather Stewart (heather.stewart@allies-llc.org)

Please DO NOT send referral packets prior to Allies requesting them. This leads to lots of unnecessary confusion.
